Information Collection and Use

Information
Collection Lighthouse Agency, Inc. is the sole owner of the information collected on www.lighthouseinsurance.com. Lighthouse Agency Inc. collects information from our customers at several different points on our web site.

Registration
In order to use this web site, a user must first complete the registration form. During registration a user is required to give contact information such as name and email address. We use this information to contact the user about services on our site or to provide insurance quotes. Providing demographic information such as income level and gender, and unique identifiers (such as username and password), is optional for the user but encouraged so we can provide more detailed information. We require a user's social security number to provide detailed insurance policies and prior to purchase of said policies.

Order
We request information from our visitors for our online quote forms and for any Lighthouse sponsored contests. Customers must provide contact information such as name, email, and address and financial information (credit card number, expiration date). This information is used for billing purposes and to fulfill customer's requests for insurance policies. If we have trouble processing an order, we use that information to contact the customer.

Application Information
This is information we receive from you on applications or other forms, by phone, and online. It includes but is not limited to your name, address, and telephone number, driver's license number, social security number, date of birth, length of employment, gender, marital status, prior insurance information, home ownership status, length of residency, vehicle descriptions, miles driven and vehicle use, other drivers, and driving history.

Transaction Information
This is information about your transactions with us, our affiliates, or others. It includes but is not limited to your insurance coverage selections and premiums, payment and claims history, and information necessary for billing, payment and underwriting. It may also include additional information obtained while adjusting, investigating, and settling insurance claims.

Consumer Report Information
This is information we receive from a consumer reporting agency. It is used to confirm or supplement Application Information so that we may have an insurer underwrite your insurance and provide an accurate premium quote for one of our insurance underwriting companies. It includes motor vehicle reports (MVR's), claims history reports (CLUE), additional driver disclosure reports (ADD) and credit report information. We will disclose Consumer Report Information only as we deem necessary to quote or service your insurance policy, and as permitted or required by law.

Information Use

Profile
We store information that we collect through cookies, log files, and user registrations to create a profile of our customers. A profile is stored information that we keep on individual customers that details their preferences. Consequently, collected information is tied to the customer's personally identifiable information to provide offers, insurance quotes and to improve the content of the site for the customer. This profile is used to tailor the customer's subsequent visits to our web site, and to direct pertinent marketing promotions to them. We do not share customer profiles with other insurance brokerage firms, but information will be reviewed by the providers with whom we work. Customer profiles are shared with your personally identifiable information.

Cookies
A cookie is a piece of data stored on the user's computer tied to information about the user. The use of a cookie is in no way linked to any personally identifiable information while on our site. Cookies are used via session management. By setting a cookie on our site, customers would not have to log in a password more than once, thereby saving time. If customers reject the cookie, they may still use our site, however, the customer is limited in some areas of our site such as contest entries.

Third Party Advertising
The ads appearing on this web site are delivered to customers through banner ads placed throughout the site. Log Files
Like most standard web site servers we use log files. This includes Internet protocol (IP) addresses, browser type, platform type, date/time stamp, and number of clicks to analyze trends, administer the site, track user's movement in the aggregate, and gather broad demographic information for aggregate use. IP addresses, etc. are not linked to personally identifiable information. IP addresses are tied to personally identifiable information to enable our web-based service. We use a tracking utility called Webtrends that uses log files to analyze user movement.

Communications from the Site

Special Offers and Updates
We send all new customers a welcoming email to verify their password and user name. Out of respect for the privacy of our customers, we present the option to not receive these types of communications.

Service Announcements
On rare occasions it is necessary to send out a strictly service related announcement. For instance, if our service is temporarily suspended for maintenance, we might send customers an email. Generally, customers may not opt-out of these communications, though they can deactivate their account. These communications are not promotional in nature.

Customer Service
We communicate with customers on a regular basis to provide requested services and in regard to issues relating to their account. We reply via email, mail or phone, in accordance with the customer's wishes.

Sharing

Though we make every effort to preserve customer privacy, we may need to disclose personal information when required by law wherein we have a good-faith belief that such action is necessary to comply with a current judicial proceeding, a court order or legal process served on our web site. We partner with a group of insurance companies to provide our customers with the lowest insurance quotes possible. When the customer signs up for these particular services, we share names, and contact information such as address, phone number and email address, with our insurance providers. These third parties are not allowed to use personally identifiable information except for the purpose of providing insurance quotes

Choice/Opt-out
Customers who no longer wish to receive promotional communications may opt-out of receiving these communications by clicking on the unsubscribe link at the bottom of the email.

Contests
From time-to-time our site requests information from customers via contests. Participation in these contests is completely voluntary and the user therefore has a choice whether or not to disclose the requested information. Customer's personally identifiable information is not shared with third parties unless we give prior notice and choice. Though we may use an intermediary to conduct these contests, they may not use customer's personally identifiable information for any secondary purposes.

ID and Address Verification
We use third parties to verify customer address and or driving records. This is a necessary action because it ensures that the customer is receiving the most accurate quote from our customer service representative.

Department of Motor Vehicles
We will verify customer information through the New York State Department of Motor Vehicles. This is a common practice for developing insurance policies.

Correcting/Updating/Deleting/Deactivating Personal Information
If a customer's personally identifiable information changes (such as zip code, phone, email or postal address), or if a customer no longer desires our service, we provide a way to correct, update or delete/deactivate customers' personally identifiable information. This can usually be done at the member sign in page or by emailing our Customer Support at customerservice@lighthouseinsurance.com.

Notification of Changes

If we decide to change our privacy policy, we will post those changes to this privacy statement, the homepage, and other places we deem appropriate so our customers are always aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it. We will use information in accordance with the privacy policy under which the information was collected.

If, however, we are going to use customer's personally identifiable information in a manner different from that stated at the time of collection, we will notify customers via email. Customers will have a choice as to whether or not we use their information in this different manner.
